Based on the findings of an unannounced onsite Medicare recertification and State relicensure survey completed June 16, 2021, Concordia Hospice of Washington was found to be in compliance with the requirements of 42 CFR, Part 418.113, Subpart D, Conditions of Participation: Hospice Care - Emergency Preparedness.
